Vaccine Policy Statement –
We, the entire providership of FCP, want to reinforce our strong support, trust and belief in our current national vaccine program and strongly support the CDC and AAP guidelines for proper and timely vaccinations. We feel that vaccinations for all our children is a centerpiece of comprehensive medical care. We further want to emphasize our belief that: 1) vaccines are safe, effective and not associated with any neurodevelopmental consequences and, 2) delaying or spreading out vaccines has no justification in current scientific thinking and simply poses undo risk to children.
Our vaccine schedule is in alignment with current CDC and AAP recommendations. We do not support altered / split / spread out vaccine schedules nor allow unvaccinated children to join our practice unless there is a medical contraindication. Click to view the CDC’s recommended vaccine schedule.
We hope that for the vast majority of our families, you will find this statement reassuring. We want to protect your child and all the children around you in our office and in our community. We want you to know that protecting your child from preventable diseases is one of our highest priorities – not only for your family but for our community. That is our mission and vision of comprehensive medical care.
Our office offers a medication refill line. We ask for 48 business hours to process medication refill requests. Please note controlled substance refills are handled Monday through Fridays and are routed to the treating provider. Please request refills for controlled medication a minimum of 3 to 5 days prior to running out.
We are no longer offering allergy shot administration at our practice. We recommend patients have their allergy injections at their allergist office.
Missed Appointment Policy
Patients are asked to arrive 10 minutes before their scheduled appointment time in order to complete the check-in process. Patients arriving more than 20 minutes late may be required to reschedule their appointment to the next available opening consistent with the type of appointment requested. Only acutely ill children will be worked into the providers schedule later the same day.
As a courtesy to both your provider and other families with sick children, we ask that you cancel any scheduled appointment 24 hours in advance so that others may utilize this time. Failure to provide at least 3 hours notice will result in a missed appointment fee of $30 and will be considered a NO SHOW. More than 2 NO SHOW / missed appointments may result in dismissal from the practice.
Form Fee Policy
School, Camp and Sport Forms
As our practice is growing, so is the number of requests to complete forms (school, daycare, summer camps, fall sports, and college). To help us better serve you in the completion of this task, please be aware of the following information:
There is a $10 fee associated with forms. This covers the administrative time necessary for review of relevant information and completion of the form. In the event your circumstances require completion of forms in less than 7 days we can offer an expedited service. The cost for the expedited service is $25 payable at the time the forms are presented, and you will receive your forms within 48 hrs. In addition, we will waive the fee if forms are presented during the well visit or sports physical. This does not apply to sick visits.
Please allow 7-10 business days for completion of your forms. We are usually able to complete one routine form (such as Frederick County Public School Exam Form) at the time of your wellness visit. If you have multiple forms that require completion, or if there is more detailed information needed to complete your form, then you will have to leave your forms for later return. Again, we will waive the form completion fee if forms are presented during the well visit or sport physical.
We will not complete any form that does not have the parent section filled out. By signing the form, we are verifying that we have reviewed the information contained in the form and have done the appropriate medical examination and screening tests necessary. We cannot do this if we don’t have all of the necessary information.
A wellness assessment may be required before some forms can be completed. In order to determine if your child is medically cleared to participate in camps or sports, it is recommended that s/he has an annual wellness assessment. Some entities may require an evaluation by a medical provider within 6 months of the start of an activity. We ask parents to please be aware of the requirements for their child’s specific activity and to plan a visit accordingly. You will need to have a wellness assessment in the 12 months prior to start of the activity, before we can complete a form asking for medical clearance for a physical activity. Please plan ahead so that you can have your wellness assessment with the medical provider of your choice.
You must pay your copayment and account/family balance at the time of your visit. If you are unable to pay your account balance at the time of a preventative visit, we may reschedule you until you are able to make the copayment or account balance payment. We will not reschedule sick visits, but you are still responsible for paying for services. Please note the parent/guardian who attends the visit is required to pay the copayment and/or balance at the time of service or arrange pre-payment to be made by the responsible party before the visit.
If you have deductible and/or coinsurance associated with your insurance plan, then you must place a payment card on file with us. The card can be a credit/debit card, a Health Savings Account (“HSA”) card, or a Flexible Spending Account (“FSA”) card. Once we have made the claim to your insurance company and the extent of your benefit is determined, we will charge your card on file for the difference.
At this time Frederick County Pediatrics accepts payment in the form of cash, debit/credit cards (include HAS or FSA) and personal checks. Any check returned unpaid will be assessed a $35 fee, and we may refuse to accept checks as payment after a returned check.
We understand that sometimes finances are difficult for families. If you are unable to pay your full balance, then Frederick County Pediatrics will assist you to structure a payment plan. Payment plans will spread your balance owed over the course of three months. If you need more time in your payment plan we are pleased to offer Care Credit. If you are interested in using Care Credit, you must contact the Practice Administrator. Frederick County Pediatrics has no obligation to agree to any payment plan.
If you do not have a payment plan and you have not paid your balance due for more than 90 days, Frederick County Pediatrics will place you into “collection status.” Accounts that are in collection status may be referred to a collections agency to collect on the balance owed. If a family is in collection status, then Frederick County Pediatrics may choose not to see your child(ren) until the balance is paid in full. Frederick County Pediatrics may dismiss you from the practice if you are in collection status.
After-Hours Medical Advice
We are pleased to offer after-hours medical advice provided by our own clinicians. When our office is closed, one of our providers can be reached by our relay phone system. We ask that any calls after hours be used for emergencies only, such as asking if a trip to the ER is warranted. To reach our on-call provider, call our office phone number and follow the prompts. You will need to leave a voice message and our relay system will automatically call our doctors. Please note that the phone you use to call out will be the phone # the provider will call you back on.
We do not accept requests for medication refills through our after-hours call service. If you need a refill, please leave a message on our refill line, which will be checked the next business day.
We do not call in medication after-hours nor diagnose conditions over the phone. Depending on the symptoms your child is experiencing, you may be instructed to make an appointment the next day for an exam.
All medical advice given during after-hours is billable; a claim will be submitted to your insurance. Insurance policies vary widely in coverage for after-hours calls. You may be assessed a fee by your plan.
