Thank you for your interest in joining our practice!
In order to register with our practice, please complete the 4 required digital documents below. In an effort to ensure we are the right fit for new families, we cannot accept new families who have not completed the required forms. Upon completion of the new forms, one of our staff members will reach out to you.
Dr. Lee does offer a complimentary prenatal meet and greet once per month. This includes a presentation reviewing both your upcoming delivery experience, follow-up care with our office, as well as time for questions. This is not required for registration!

Many new parents have questions about vaccines. Please be aware that we support and view childhood vaccines as a cornerstone of preventative, comprehensive care, and encourage you to review our vaccine policy statement and view the CDC’s recommended vaccine schedule.
